Heat, cold, testosterone, and what sperm need

The internet says heat and cold will "optimize" your fertility. Your testicles would like to file a complaint.
Somewhere along the way, self improvement turned into a competitive sport. There's sleepmaxxing, looksmaxxing, testmaxxing, and roughly forty other maxxings, each with a guy on your feed explaining why you're doing life wrong.
Fertility got pulled into it too. Scroll long enough and you'll find men icing their testicles, chewing raw garlic like it's trail mix, taking eleven supplements before breakfast, and alternating between a 190 degree sauna and a barrel of ice water in pursuit of championship sperm.
So does any of this work? Some of it does nothing. Some of it might be backfiring. Let's sort it out.
Mistake number one: thinking testosterone and fertility are the same thing
They are not. This is the misunderstanding that everything else is built on top of.
Testosterone matters for male reproduction, absolutely. But your blood testosterone level is not the number that makes sperm. Sperm production depends on testosterone inside the testicle, at concentrations far higher than anything circulating in your bloodstream. Adding testosterone from outside the body does not raise that internal level.
It does the opposite. Taking testosterone tells your brain that there's plenty around, so your pituitary gland stops sending the signals (LH and FSH) that keep the factory running. Internal testicular testosterone crashes. Sperm production drops, sometimes to zero.
This isn't a rare side effect. It's so reliable that researchers have studied testosterone as a male birth control method. Let that sit for a second. The thing being sold as fertility optimization has been formally tested as contraception.
Recovery after stopping is possible but slow, taking months to years, and it varies a lot between men. For some, sperm never fully returns. This is why the American Urological Association, the American Society for Reproductive Medicine, and the Endocrine Society all recommend against testosterone therapy for men who want kids now or later.
If you're currently on testosterone or steroids and want children: talk to a urologist rather than quitting cold turkey on your own. There are actual medical protocols for restarting sperm production, and doing it with supervision works better than guessing.
Why testicles hang outside the body (it's not an accident)
Sperm production works best a few degrees cooler than the rest of you. That's the entire reason the anatomy is arranged that way. It's not a design flaw. It's a cooling system.
Which means anything that keeps the area warm for extended stretches is working against the machinery.
The heat evidence
The big population studies. A 2024 Chinese study analyzed 28,267 semen samples from more than 15,000 men and found that temperature extremes in both directions were linked to worse semen quality. Lower sperm concentration, lower total count, lower motility. A larger dataset of 78,952 samples from 33,234 sperm bank donors found the same curved pattern, where the middle is fine and both ends are worse. Heat wave studies in Argentina found that men exposed during the roughly 64 to 90 day window when sperm are developing had measurably lower sperm counts and worse shape.
Hot weather isn't the same as choosing to sit in a wooden box at 185 degrees, but the direction of the signal is consistent.
The sauna study, which is the one that actually matters. An Italian team followed 10 healthy men with normal semen who did two 15 minute Finnish sauna sessions per week at 80 to 90°C (176 to 194°F) for three months.
Their sperm count and motility dropped significantly. So did mitochondrial function, which is the sperm's power supply. Even the way DNA was packaged inside the sperm head changed, and genes related to heat stress and low oxygen got switched on. Basically the cells filed a formal grievance at the molecular level.
Here's the good news, and it's genuinely good: six months after they stopped, everything had returned to normal. All of it. A systematic review of dry sauna bathing landed in the same place, noting that the one reproductive effect found was reversible.
Stacking matters. A US study of couples trying to conceive found that hot baths and hot tubs on their own barely moved pregnancy rates. But men with heavy combined exposure to multiple heat sources had moderately lower odds of conceiving. It's a cumulative load, not a single villain.
The cold evidence, which is where the theory falls apart
Reasonable assumption: if heat hurts, cold must help. Reasonable, and wrong.
Cold water immersion has decent evidence for reducing muscle soreness and helping short term recovery after hard training, plus possible benefits for stress, sleep, and general quality of life. Those studies are small and inconsistent, but the effects are plausible.
What no review has ever shown is that cold water immersion improves testosterone, sperm quality, or pregnancy rates.
In fact, in a study of 13 young men, one immersion in water colder than 4°C caused a significant drop in blood testosterone. Values stayed in the normal range, but the arrow pointed down, not up. And those large population studies found unusual cold linked to lower sperm motility and count too.
There's one real exception, and the internet has garbled it badly. Targeted nocturnal scrotal cooling has improved sperm concentration, output, motility, and shape in men with poor semen quality, and it's being studied as an actual treatment. That is a specific medical device applied to a specific area overnight. It is not the same as dunking your entire body in a stock tank while a phone films you gasping.
Also, no, ice afterward does not cancel out the sauna. There is zero evidence for that. Heat and cold are not opposing charges that neutralize each other.
One safety note the wellness feeds skip: the paper on sub 4°C immersion is literally titled around whether it's a health hazard. Sudden cold water triggers a gasp reflex and a spike in heart rate and blood pressure. Don't do it alone, don't do it in deep water, and if you have any heart condition, ask a doctor first.
What actually helps male fertility
The stuff that works is boring. It is always boring. But here it is:
Stop smoking, including vaping and heavy cannabis use. All are linked to worse semen quality.
Cut back on alcohol. Heavy drinking is associated with lower sperm counts.
Get to a healthier weight if you're carrying extra. Obesity is linked to lower sperm quality and shifts your hormone balance.
Exercise moderately. Regular activity helps. Extreme endurance training plus a daily sauna habit is not the flex you think.
Reduce everyday heat. Laptop off the lap. Long hot tub soaks less often. Heated car seats on low. If you cycle for hours, look into a better saddle.
Get checked for a varicocele. These are enlarged veins in the scrotum, they're common, they raise local temperature, and they're treatable.
Sleep. Yes, really.
Treat infections promptly. Untreated STIs can damage the plumbing permanently.
Skip the mystery supplement stack. Very little of it is supported, and some of it is contaminated with things you didn't order.
The three month rule nobody mentions
Sperm take roughly 70 to 90 days to develop. That means whatever you did in April shows up in your July test results.
This cuts both ways. Any damage from a heat wave, a fever, or a three month sauna phase takes about that long to clear. And any good change you make today won't show up in a semen analysis for about three months either. If you make changes and retest in two weeks, you've learned nothing.
Practical version: if you're actively trying to conceive, consider parking the sauna habit for three months and see what happens. It's a reversible experiment with an actual endpoint.
What about women?
Here the honest answer is that we mostly don't know.
Wellness culture aims plenty of claims at women too, usually dressed up as "hormone balance" or "egg quality." The research supporting them is much thinner. Reviews note that the effects of heat on female reproduction are underexplored in humans, and that most of what exists on cold exposure comes from animal studies. Nobody has established whether cold water immersion affects ovulation, egg quality, or conception.
Pregnancy is a separate conversation with its own considerations, and high heat exposure in early pregnancy in particular is something to raise with a doctor rather than a podcast.
The thing everyone should actually do
If you and a partner have been trying for 12 months without success, or 6 months if she's over 35, get evaluated. Both of you.
About half of infertility cases involve a male factor, and yet the semen analysis is routinely the last test anyone orders. It's a cheap, fast, non invasive test that gives you real numbers instead of vibes. You cannot optimize a variable you have never measured.
Bottom line
The problem usually isn't the sauna or the ice bath. Either one might genuinely help you sleep better, feel calmer, or recover from a hard workout. Those are decent reasons to do them.
The problem is assuming that one good thing automatically produces every other good thing. A sauna helping you relax says nothing about your sperm count. Stacking six unproven trends on top of each other doesn't add up to health. It just adds up.
And when it comes to fertility specifically, some guys are quietly maxxing the one number they were hoping to raise.
