Cardio has been hogging the microphone for 40 years. New research says your muscles deserve a turn.
Here is an uncomfortable question. When was the last time you picked up something heavy on purpose?
Not a toddler. Not a grocery bag. Not the couch you moved because you swore the lamp looked better on the other wall. Something heavy, on purpose, for the specific reason that it would make you harder to kill.
If your answer is "hmm," you are in excellent company. You are also, according to a new study, leaving a startling amount of health on the table.
The study
Researchers at the University of Queensland followed more than 8,000 adults in Brisbane, Australia, all between the ages of 40 and 64. They tracked these people for nine years across five surveys, from 2007 to 2016, asking a simple question over and over: what kind of exercise are you actually doing? Then they watched who got sick.
The people who did both types of recommended exercise, cardio and strength training, came out dramatically ahead of the people who did neither.
The numbers, for the group doing both:
50% lower rate of type 2 diabetes
24% lower rate of high blood pressure
23% lower rate of cancer
"People who consistently met the recommended guidelines for both aerobic and muscle-strengthening activities were less likely to develop type 2 diabetes and hypertension and may also have had a lower risk of heart disease and some cancers than those who met neither guideline," said Dr. Gregore Mielke, a behavioral epidemiologist at UQ's School of Public Health.
Cut your diabetes risk roughly in half. That is not a supplement ad. That is a squat rack.
This is not a one-off finding
One study is a rumor. A pile of studies is a pattern, and the pile here is getting tall.
A review of prospective cohort studies found that just one or two strength sessions a week, roughly 60 to 150 minutes total, was linked to about 20% to 25% lower risk of cardiovascular disease, about 30% lower risk of type 2 diabetes, and 15% to 20% lower risk of dying from cancer.[3]
Read that again, because the time commitment is the shocking part. Sixty minutes. A week. That is one episode of a prestige drama you have been meaning to finish.
A separate analysis in JAMA Internal Medicine found that people who hit the strength guidelines plus the cardio guidelines had lower rates of death from all causes, from heart disease, and from cancer.[1]
And a pooled analysis of three large cohorts, followed for an average of 19 years, found that 1 to 2 hours of resistance training per week was linked to a 27% lower risk of developing diabetes. The kicker: that benefit held up even in people who were already doing plenty of cardio.[5] Strength training was not redundant. It was adding something cardio was not providing.
The American Heart Association's 2023 statement puts the diabetes number at 17% lower, and notes that most of the benefit shows up in the first 60 minutes per week.[2] The first hour is where the magic lives. Everything after that is bonus.
One honest caveat before you go buy a barbell
More is not infinitely better. Some evidence hints that the benefits for overall mortality and heart disease may flatten out, or even reverse slightly, past about 2.5 hours of strength training per week.[3]
Nobody fully understands why yet, and the finding is not rock solid. But it is a useful reminder that the goal here is not to become a person who lives in a gym. The goal is to become a person who occasionally visits one.
Why does this work?
Your muscles are not just there to look nice in a t-shirt. They are the largest place in your body that stores and burns blood sugar. Think of muscle as a parking garage for glucose.
When you strength train, a few things happen:
Your muscles get better at taking sugar out of your blood. They do this using a protein called GLUT4, which works like a door on the muscle cell. Training builds more doors and opens them faster. More parking spots, better traffic flow, less sugar circling the block.
You build and protect lean muscle. This matters enormously as you age, because after about 30, adults lose muscle steadily unless they do something about it. That loss is a slow-motion emergency almost nobody notices until they cannot get out of a chair.
You lose the dangerous kind of fat. Not the fat you can pinch, but visceral fat, the stuff packed around your organs. That is the fat most tied to heart disease and diabetes.
Inflammation and blood fats improve. Chronic low-grade inflammation is involved in nearly every disease on this list.
The measurable results back this up. Meta-analyses show resistance training drops systolic blood pressure by roughly 4 to 7 points, and lowers HbA1c (a three-month average of blood sugar) by about 0.3% to 0.6% in adults with type 2 diabetes.[6][7] A 0.5% drop in HbA1c is in the neighborhood of what some medications deliver. From lifting things.
And combining cardio with strength beats either one alone for blood sugar control and body composition.[8][9] Cardio and weights are not rivals. They are a duo, like peanut butter and jelly, or like two coworkers who each cover the thing the other one forgets.
So why is nobody doing it?
Because for 40 years, public health messaging has been essentially one long commercial for jogging.
The official guidelines have said otherwise for years. The 2018 Physical Activity Guidelines for Americans and the 2020 WHO guidelines both call for:
150 to 300 minutes of moderate cardio per week (or 75 to 150 minutes of vigorous), plus
muscle-strengthening activities hitting all the major muscle groups on 2 or more days per week[4][1]
Australian guidelines say roughly the same thing.
That second bullet point has been sitting there quietly, getting ignored, like the vegetable side dish at a barbecue.
In the UQ study, about half of participants met the cardio guideline. Only 24% met both. Roughly 30% met neither. In the US, fewer than one in three adults report doing the recommended twice-weekly strength work.[2]
"This research suggests public health initiatives should also place emphasis on encouraging adults to incorporate strength-based activities into their weekly routines to support healthy aging," Mielke said.
Lead author Anh Tung Pham, a Ph.D. candidate, pointed out why middle age is the moment that matters. Australia's 2022 National Health Survey found about 60% of adults aged 45 to 64 already have at least one chronic disease, and 30% have more than one. "This provides evidence to stress the value of this combined approach for chronic disease prevention," Pham said.
Translation: the alarm is going off in your forties. Strength training is one of the few snooze buttons that actually works.
The fine print (because you deserve it)
Good science reporting means telling you what a study cannot prove, so here it is.
This is observational, not an experiment. Nobody was randomly assigned to lift weights. Researchers watched what people did and tracked what happened. That means these are associations, not proof of cause and effect.
People who lift weights are different in other ways. They may sleep better, drink less, eat differently, have more free time, or have more money. Researchers statistically adjust for these things, but adjustment is a good tool, not a perfect one.
The exercise data was self-reported. People are famously generous when describing their own workouts.
"50% lower risk" is relative, not absolute. If your baseline risk of something is 4%, cutting it in half takes it to 2%. That is a genuinely great deal. It is not the same as a 50-point drop.
None of this makes the findings weak. It is a large group, followed for nine years, and it lines up with a mountain of other research and with mechanisms we understand at the cellular level. It just means the honest sentence is "strongly linked to," not "guaranteed to."
Okay. How do I actually start?
Here is the part most articles leave out.
You do not need a gym. Body weight counts. Push-ups, squats, lunges, sit-ups, planks, and step-ups all qualify. So do resistance bands, which cost about as much as a sandwich and fit in a drawer.[4][9]
"All major muscle groups" means roughly six movements. Push something. Pull something. Squat. Hinge at the hips. Carry something heavy. Brace your midsection. Cover those, and you have covered your body.
Two days a week is the target, not a starting point you have to leap to. Start with one. One session a week beats the zero sessions a week you have been doing while reading articles like this one.
Twenty to thirty minutes is plenty. Remember, most of the benefit shows up in the first hour per week.
Aim for effort, not exhaustion. A useful rule: stop each set when you could probably do 2 or 3 more reps with good form. Somewhere around 8 to 15 reps per set is a fine zone for most people.
Then make it slightly harder over time. This is the one non-negotiable principle. Add a rep, add a little weight, or slow the movement down. Muscles respond to being asked for more. Doing the exact same 10 push-ups forever is a hobby, not a stimulus.
Do not quit cardio. The whole point of the research is that the combination wins. Walking counts as cardio, by the way, and a brisk 30-minute walk most days gets you there.
Talk to a doctor first if you have heart disease, uncontrolled high blood pressure, are pregnant, have a recent injury or surgery, or have not exercised in a long time. This is standard advice, not a scare tactic. A brief conversation is cheap insurance.
The takeaway
You cannot buy a supplement that cuts your diabetes risk in half. You cannot download an app that lowers your blood pressure 5 points. But there is a pile of evidence suggesting that picking up heavy objects a couple of times a week, for less time than you spend deciding what to watch, moves the needle on the diseases most likely to shorten your life.
