
Okay. Deep breath. We need to talk about poop.
Specifically, we need to talk about a real medical trial where researchers took gut bacteria from healthy donors, packed it into capsules, gave those capsules to people with terrible insomnia, and then measured what happened while they slept.
The people slept better. Measurably better. In a lab, hooked up to machines, no guessing involved.
I know. I had questions too. Let's start at the beginning.
First, the problem
Chronic insomnia is not "I had a rough night." The official version means you struggle to fall asleep or stay asleep at least three nights a week, for three months or longer, and it wrecks your days too.
About 10% of adults have the full-blown version. Another 15% to 20% deal with it on and off. It hits women harder than men, and the gap gets wider after age 45. It also piles up on older adults, shift workers, and anyone already managing another health problem.
This is not just about being cranky at work. Chronic insomnia is linked to higher risks of depression, high blood pressure, heart disease, and Alzheimer's. In the U.S. alone, it costs an estimated $411 billion a year in lost work and medical bills.
That is a lot of money to spend on staring at a ceiling.
What we currently do about it
The best treatment we have is not a pill. It's called CBT-I, which is a structured program that retrains how you sleep and how you think about sleeping. Every major sleep organization recommends it first. It works.
There's just one catch. There are nowhere near enough trained therapists to go around. Great treatment, tiny supply.
So people end up on sleep medications instead. Those work, but they come with real baggage: dependence, next-day grogginess, and other side effects. Doctors generally treat them as backup, not the main plan.
That gap between "what works" and "what people can actually get" is why researchers keep hunting for something new. Which brings us to your intestines.
Your gut talks to your brain. Constantly.
Here's the part that sounds made up but isn't.
The trillions of bacteria living in your intestines are in constant conversation with your brain. Scientists call this the gut-brain axis, and it runs on several channels at once. There's the vagus nerve, which is basically a direct phone line from gut to brain. There's your stress hormone system, the one that controls cortisol. There are immune signals, the chemical messages your body uses to manage inflammation. And there are chemicals your bacteria actually make, including short-chain fatty acids and GABA. GABA might sound familiar: it's the calming brain chemical that a lot of sleep and anxiety medications target.
Your gut bacteria are, in a small but real way, manufacturing mood and sleep chemistry.
And when researchers looked at real people, the pattern held up. People with more diverse gut bacteria tend to sleep more efficiently and spend less time awake in the middle of the night.
The bad news is that it runs both ways. Losing sleep damages your gut bacteria, and damaged gut bacteria mess with your sleep. It's a loop that feeds itself, which is exactly the kind of problem where you'd love to break in from the outside.
The trial that made everyone pay attention
In July 2026, a research team at Peking University published results in the Journal of Internal Medicine, and this one was done properly. Randomized, double-blind, placebo-controlled, multiple sites. That's the good stuff.
They enrolled 80 adults with chronic insomnia and split them into two groups. One group got a short course of antibiotics (to clear space, essentially) followed by capsules containing healthy donor microbiota. The other group got placebo capsules.
Then, and this is the important bit, they measured sleep with overnight polysomnography. That's a full sleep lab study. Not a questionnaire. Not "how do you feel." Actual electrodes measuring actual sleep.
Here's what they found. Sleep efficiency improved by 13.9 percentage points in the transplant group compared to placebo. Sleep efficiency means the share of time in bed you're actually asleep, and nearly 14 points is not a rounding error: that's the difference between lying awake for an hour every night and not. People woke up less during the night. The improvements lasted, with sleep quality scores staying better from two months out to six months. And the side effects were mild and went away on their own, with nobody having a serious adverse event.
They also tried adding a supplement blend on top for some participants. It did nothing extra. Sorry, supplement aisle.
One more finding worth flagging. When they looked at who improved and who didn't, the difference wasn't in how their gut changed afterward. It was in what their gut looked like before treatment. That hints at a future where a doctor could test you first and know whether this would even work for you.
This isn't the only study
The Peking trial is the strongest evidence, but it's got company.
A 2023 real-world study of 33 patients found insomnia severity scores dropped from 17.3 to 5.4 in four weeks. Sleep quality scores fell from 14.6 to 6.6. Anxiety and depression scores improved too, and about 76% of patients hit the main goal.
A 2024 study using a filtered version of the procedure found better sleep latency (how fast you fall asleep) and longer sleep, with bigger gains after more rounds of treatment.
And in people with long COVID, a 2024 trial found insomnia went into remission in 37.9% of the treated group versus 10% of controls. Their cortisol levels dropped too, which fits neatly with the stress-hormone pathway.
What about just taking probiotics?
Fair question. Probiotics are the easy, buy-them-at-the-store version of this idea. So do they work?
Kind of. Barely.
A 2025 review pooling 39 trials and over 4,000 people found probiotics improved sleep quality scores a little and added roughly 14 extra minutes of sleep per night. The researchers themselves rated the quality of that evidence as low, and 14 minutes is not going to change your life.
A smaller review looking only at people with actual insomnia found somewhat bigger improvements in how people rated their sleep, but no measurable change in objective sleep numbers.
Translation: swapping in an entire ecosystem seems to do more than sprinkling in a few species and hoping. Which, if you think about it, makes sense.
Now the part where I pump the brakes
I've made this sound great, so here is the honest accounting.
Eighty people is small. Encouraging, not conclusive. Plenty of exciting small trials have fallen apart when someone ran a bigger one.
It was done at centers in China. No knock on the research, but we don't yet know if the results hold across different populations, diets, and genetics.
There's a design problem. The treatment group got antibiotics first. The placebo group didn't. So we can't fully rule out that the antibiotics themselves did some of the work. That's a real gap, and future trials need to close it.
Nobody knows the details yet. Who makes the best donor? What dose? How often? Does it still work after a year? All open questions.
The one thing you should absolutely not do
🚫 Do not attempt a DIY fecal transplant at home. Ever.
This is a real medical procedure with real screening requirements. Donor stool is tested extensively for infections and pathogens for very good reasons, and people have been seriously harmed, some fatally, by unscreened transplants. The internet has instructions. The internet is wrong about this. There is no safe home version, no matter how carefully you think you've vetted a donor, because the dangerous pathogens don't announce themselves. If you have chronic insomnia today, the boring answer is still the right one: ask a doctor about CBT-I, including the app-based and online programs that exist precisely because there aren't enough therapists.
The bottom line
We are watching a genuinely new idea get its first serious test, and it passed.
For the first time, a rigorous trial has shown that changing someone's gut bacteria produces a measurable, machine-verified improvement in how they sleep. It's not ready for your doctor's office. It might never get there. But it's a real result, not a headline.
The strangest part isn't the method. It's what the method implies: that some of what happens in your head at 3 a.m. is being negotiated somewhere much further south.
