Hearing loss is extremely common in older adults, yet under-treated. About:

• 1 in 5 people ages 65–74 have hearing loss

• Over half of those over 75 are affected

But fewer than one-third of older adults who could benefit from hearing aids actually use them.

Cochlear implants growing more common

For people whose hearing aids no longer provide clarity, cochlear implants can make a major difference — even at older ages.

Once rare for older adults, implants are now routine for people over 80, with rapidly increasing use. A 2023 study showed implantation is growing fastest among the 80+ age group.

Medicare expansion has opened access

Historically, Medicare covered implants only for people who performed below 40% on word recognition tests. In 2022, coverage expanded to people scoring up to 60%, making far more older adults eligible.

Still, fewer than 10% of eligible adults receive implants.

What the procedure and recovery are like

Cochlear implantation:

• Is an outpatient surgery lasting 2–3 hours

• Includes placing an internal receiver and electrodes

• Requires an external processor

• Is activated 2–3 weeks later

When first activated, sound often seems robotic. The brain adapts over weeks to months.

Typical outcomes:

• By 1–3 months → major clarity improvements

• By 6 months → most of the benefit is achieved

• By 12 months → continued gradual gains

A major Johns Hopkins study found adults 65+ gained about 50 more correct words out of 100 on sentence tests — similar to improvements seen in younger adults.

Older adults often report:

• Improved communication

• Reduced social isolation

• Better cognitive test scores

Even adults in their late 80s and 90s benefit, including those with mild cognitive impairment.

Limitations

• Some dizziness or nausea after surgery

• Technology can be challenging for some users

• Not ideal for extremely noisy environments

• Music may sound unnatural

• People with relatively good speech understanding may not find the benefit worth the effort

Still, for many older adults struggling with clarity despite hearing aids, cochlear implants offer a transformative improvement.
